Intensive Behavioral Health Navigator – Nebraska CCBHC

Summary Description of Work Respond to clients who may have intensive behavioral health needs in the community. Reach out to individuals, screen for needs, build rapport, connect to resources, and provide intensive case management for a small caseload.
Compensation Between $20.92 and $24.96 per hour, based on total years of relevant experience.
Work Schedule Monday through Thursday 8:30 am – 5:00 pm, Friday 8:30 am – 2:30 pm (37.5 hours per week).
Minimum Qualifications
Bachelor’s Degree in Counseling, Social Work or related field preferred.
Three years’ experience with a similar population preferred.
Valid driver’s license and acceptable driving record.
Essential Duties and Responsibilities
Participate in community outreach, screen clients for presenting problems including mental health, substance use, physical health, housing, and other needs.
Utilize a strengths‑based approach when providing immediate community behavioral health intervention, demonstrating a trauma‑informed format of de‑escalation.
Assist clients in identifying service options and making referrals, utilizing extensive knowledge of community resources.
Advocate for clients with other HFS programs, police, court, hospitals, and community stakeholders.
Assist clients in problem‑solving barriers to treatment, including benefit management needs.
Understand the culture of homelessness, poverty, substance use disorders, severe and persistent mental illness, and associated symptoms.
Check client benefits with insurance companies.
Ensure Medicaid/insurance coverage and client understanding of coverage.
Enter collected data into the computer database, schedule intake/follow‑up appointments and update records.
Process “Release of Information” forms to obtain necessary clinical data.
Maintain records and reports for statistical documentation of referrals and client tracking.
Prepare monthly statistical reports.
Maintain all reports required of the position.
Display comfort in dealing with stressful individual/family dynamics.
Display a courteous and caring attitude at all times to clients, volunteers, and visitors.
Cooperate and collaborate with program area staff, volunteers, and other agency staff.
Be dependable and punctual regarding scheduling and attendance.
Abide by all specific program and agency procedures, policies, and requirements.
Develop personal and program-related skills through participation in training opportunities.
Create, maintain and share a dynamic self‑care plan.
Strive to connect the agency with the larger community to contribute to fundraising and outreach.
Perform other program‑related duties as assigned.
