Care Transition Coordinator
Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT)
– Vertus HealthCare is hiring a driven clinician to serve as a Care Transition Coordinator. This relationship-focused, growth-oriented role focuses on building partnerships, driving referrals, and improving patient outcomes through seamless post‑acute care transitions.
Responsibilities
Drive referral growth by building strong relationships with hospitals, health systems, inpatient facilities, and physician groups.
Serve as Vertus HealthCare clinical liaison, representing our services and value in transitional care programs.
Guide patients through safe, timely transitions from acute care to home health.
Assess, plan, coordinate, and monitor patient‑centered transition plans using evidence‑based best practices.
Promote adherence to post‑acute care plans and ensure services are initiated and completed.
Manage admission activity within an assigned territory while meeting or exceeding referral and admission goals.
Conduct ongoing quality assurance visits with referral sources to support satisfaction and long‑term partnerships.
Track performance and outcomes while supporting growth initiatives.
What We’re Looking For
Licensed RN, PT, or OT in the state of employment.
Graduate of an accredited nursing or therapy program.
2-3+ years of clinical experience, preferably in home health, hospice, or post‑acute care.
Strong sales mindset with the ability to influence, negotiate, and close referrals.
Knowledge of transitional care best practices and post‑acute regulations.
Excellent communication, presentation, and relationship‑building skills.
Experience managing a territory, prioritizing leads, and executing sales plans.
Self‑motivated, results‑driven professional who works independently.
Previous healthcare sales or liaison experience preferred.
