Pre‑Admission Testing Registered Nurse Navigator – Surgical Home – Full Time – Days (0800‑1600)
Job Summary
The RN – Nurse Navigator acts as the primary point of contact and is responsible for monitoring and coordinating care for cancer patients and families. Individualized assistance is offered to patients, families, and caregivers to help overcome healthcare system barriers and facilitate timely access to quality health and psychosocial care from pre‑diagnosis through all phases of the cancer.
Core Job Responsibilities
Act as a single point of contact for patients and their families, facilitating appointments for consults and support services within established service standards.
Provide information throughout the continuum of care, including educating the patient, their family, and caregivers regarding their diagnosis and treatment options.
Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical trials.
Coordinate care logistics for patients and communicate among providers.
Assist in making the care seamless, continuous, and comprehensive.
Assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs.
Assist patients to overcome barriers to care, e.g., transportation, childcare, financial counseling.
Provide assessment of psychosocial needs and refer to social worker as needed.
Coordinate/participate in multidisciplinary cancer conferences, developing concise patient summaries for use by the team and documenting recommendations made using standardized care protocols in accordance with nationally recognized care guidelines.
Collaborate with the cancer registry to collect data, track outcomes, and support strategic planning processes.
Conduct outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, social services).
Serve as a resource for community educational events, such as health fairs, screenings, and symposiums.
Education / Experience Requirements
5 years experience in an area of specialty.
Licensure / Certification Requirements
Active NYS Licensed RN
BLS
Preferred Qualifications
BSN and OCN
