Registered Nurse – PRN – Community Health – Ft. Wayne – Ambulatory
We are seeking an experienced Registered Nurse for a PRN supplemental position. This role is not eligible for benefits. If you are an upcoming or recent nursing graduate, please apply through our pipeline requisition.
IU Health is dedicated to advancing healthcare for all. As a RN in our Community Health program, you will collaborate with multidisciplinary teams, provide clinical care, health screenings, and health education to vulnerable populations. You will also offer health coaching, counseling, and support in home, community, and outreach settings. This position requires travel to homes, schools, and community centers; a valid driver’s license and reliable transportation are required.
Schedule: PRN; primarily evenings and weekends.
Responsibilities • Provide day‑to‑day clinical knowledge, coaching, and health screenings to vulnerable populations.
• Deliver health screenings (blood pressure, diabetes, lipids, social determinants of health) and assist with immunizations.
• Support care navigation and assist individuals, families, and communities in accessing resources such as health insurance, food, housing, and quality care.
• Coordinate operations and oversight for Community Health Workers (CHWs) and frontline staff delivering community‑based services.
• Serve as a bridge between community members and healthcare, government, and social service systems.
Qualifications
Associates of Nursing (ASN) required; Bachelor of Nursing (BSN) preferred.
Active Registered Nurse (RN) license in the state of Indiana or a valid Nursing Licensure Compact (NLC) RN license.
Graduation from a nationally accredited nursing program.
Basic Life Support (BLS) certification through the American Heart Association; additional advanced life support certifications may be required per unit or department specialty.
Valid driver’s license and reliable transportation.
