Job Summary
The Breast Imaging Patient Navigator is responsible for guiding patients through the healthcare system by providing education, advocacy, and coordination of care for individuals undergoing breast imaging and related procedures. This role ensures timely access to diagnostic services, fosters communication among care providers, organizes educational outreach, and facilitates a seamless continuum of care. The Navigator works closely with departments such as Radiology, Surgery, Oncology, and the Cancer Center to support high-quality, patient-centered care.
Essential Functions
Assist breast care patients with navigating the healthcare system and accessing diagnostic and treatment services
Coordinate episodes of care by ensuring timely scheduling, follow-up, and pre‑procedure work‑ups
Serve as a liaison between patients and providers across Radiology, Surgery, Oncology, and ancillary services
Provide standardized and current patient education materials on breast imaging and related procedures
Lead or support community outreach and provider‑based education seminars to promote awareness and access
Participate in staff education and training efforts across departments
Collect, track, and report key performance measures including patient satisfaction, outcomes, and timeliness of care
Collaborate in continuous quality improvement and present performance data to appropriate committees
Maintain flexible hours to meet patient and physician needs, including schedule adjustments as necessary
Knowledge / Skills / Abilities / Expectations
Strong clinical reasoning, communication, and documentation skills
Ability to interpret clinical data and apply problem‑solving strategies
High degree of initiative, creativity, and professional judgment
Competency in computer systems and electronic medical records
Strong interpersonal skills with a commitment to compassion and patient advocacy
Qualifications Education
Bachelor of Science in Nursing (BSN) or Master of Social Work (MSW) required
Licenses / Certifications
Current RN license or social work licensure required
Advanced certification in breast patient navigation required within the time frame required by facility policy
Experience
Minimum of 2 years' experience in a healthcare setting, preferably in Oncology or Case Management
Demonstrated experience working effectively with physicians and multidisciplinary teams
