Position specific details:
The
Behavioral Health Case Manager II
will work with our behavioral health members diagnosed with mental illness disturbances. The role will require in‑person meetings with members as per contractual requirements. Candidates residing in Orange, Osceola, Seminole, Miami‑Dade, Broward, or Palm Beach counties are encouraged to apply.
The
Behavioral Health Case Manager II
is responsible for performing case management telephonically and/or by home visits within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs. The candidate will be a subject‑matter expert in targeted clinical areas such as Eating Disorders (ED), Maternity Alcohol/Drug, Autism Spectrum Disorders (ASD), and more.
How You Will Make An Impact
Respond to more complex cases and account‑specific requests.
Use appropriate screening criteria, knowledge, and clinical judgment to assess member needs.
Conduct assessments to identify individual needs and develop a specific care plan to address identified objectives and goals.
Monitor and evaluate effectiveness of the care plan and modify it as needed.
Support member access to appropriate, quality, and cost‑effective care.
Coordinate with internal and external resources to meet identified member needs and collaborate with providers.
Serve as a resource to other BH Case Managers.
Participate in cross‑functional team projects and initiatives.
Minimum Requirements Requires MA/MS in social work counseling or a related behavioral health field, or a degree in nursing, with a minimum of 3 years of clinical experience in social work counseling involving complex psychiatric and substance abuse or substance abuse disorder treatment. Equivalent education and experience may be considered. Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist, as allowed by applicable state laws, is required. Prior experience in case management and telephonic and/or in‑person coaching with members with complex psychiatric/substance abuse and/or medical disorders is required. Managed care experience is required. For Government business, LAPC or LAMFT is also acceptable.
For associates working within Puerto Rico who are member or patient facing in a clinical setting or in the Best Transportation unit, a current PR health certificate and a current PR Law 300 certificate are required for this position.
Preferred Skills, Capabilities, and Experiences
Bilingual in Spanish and Creole is preferred.
The ability to travel to facilities is strongly preferred.
Strong communication and detail orientation skills are preferred.
Experience in health coaching and motivational interviewing techniques is preferred.
