Provides clinical case management including ongoing psychosocial assessment and treatment planning for patients within a variety of specialty areas in the hospital. Collaborates with multidisciplinary teams to provide appropriate discharge, ongoing and follow-up plans including the coordination of community and hospital resources and care.
Qualifications
Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Master's Degree in Social Work field and two years of previous work related experience. Licensure as a Clinical Social Worker by the State of New Jersey required. About Us
