Job Summary The Patient Liaison Advisor utilizes their specific clinical knowledge to provide personalized assistance to patients, families, and caregivers through navigating health care services and facilitating timely access to quality medical and psychosocial care throughout all phases of the care continuum from screening/pre‑diagnosis to treatment. The Patient Liaison Advisor works collaboratively with the multidisciplinary disease team to coordinate the patient’s plan of care and communicate with all members of the team on behalf of the patient. This nurse functions as an expert communicator over the telephone or in person, providing timely, appropriate advice and/or guidance to patients who seek assistance with health care issues. It is a complex role requiring flexibility and independent decision making as well as multi‑focal collaboration.
Hours: Monday – Friday, 8:00 am – 5:00 pm CST with extended hours as business needs dictate

Key Responsibilities
Facilitates new patient entry
Provides education and emotional support and address questions/concerns throughout all phases of the cancer continuum
Facilitates personalized care within the context of functional status, cultural consideration, health literacy, psychosocial, reproductive, and spiritual needs for patients, families, and caregivers
Develops a comprehensive and evidence‑based plan of care in collaboration with the care team, patient, family, and caregivers to optimize the patient’s treatment and care
Provides ongoing education, resources, and referrals to both internal and external providers/groups
Serves as the primary contact for patient issues and patient care barriers associated with the coordination of their care
Interacts routinely with the care team to address patients’ questions and care coordination concerns
Tracks and monitors diagnosis and treatment information, outcomes, and patient contacts for measuring and improving quality of care for the individual patient and patient population
Responsible for consistently documenting all assigned patient touch points
Provides reports and organizes data as requested
Enhances relationships within VUMC and in the community that results in more donors providing philanthropic funding
Technical Capabilities
Evidence‑Based Practice (Intermediate) : Recognizes, implements, and evaluates practice changes based on published research. Demonstrates expertise in applying evidence‑based practices to challenging and complex situations.
RN Access Patient Education (Intermediate) : Demonstrates uppermost levels of patient education in practical applications of a complex nature. Possesses mastery of knowledge, training, and expertise to be capable of successfully delivering treatment planning services across the care continuum. Takes a lead role in complex situations when there is a need to achieve results. Actively participates in outside professional organizations and forums.
Nursing Patient Assessment & Evaluation (Intermediate) : Demonstrates the uppermost levels of expertise in patient assessment and evaluations in challenging and complex situations. Conducts primary care patient interviews and physical examination. Often takes a lead role in complex patient care situations.
