Telehealth – Nurse Practitioner Responsibilities
Will primarily support patients via a video‑enabled telehealth platform while a Licensed Practical Nurse (LPN) or Certified Medical Assistant (MA) facilitates the visit in the patient’s home.
Engage attributed patients into the WellBe program and support care needs to keep patients engaged.
Facilitate staff, patient, and family decision‑making by providing educational resources.
Provide primary care via the telehealth platform, working closely with the LPN or MA in‑home with the patient.
Perform initial patient screenings/video‑enabled physical examinations and create appropriate assessment, treatment, and plan.
Collaborate with community team to complete a full patient care assessment and plan.
Order, interpret, and analyze diagnostic tests and determine need for follow‑up appointments and further treatment options per state supervisory agreement.
Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents per state supervisory agreement.
Update patient records and verify accuracy at each appointment.
Deliver patient care and complete regular patient reviews with supervising physician per state supervisory agreement.
Focus clinical care delivery to support necessary documentation/coding, HEDIS measures, engagement of patients, and managing medical expenses.
Utilize all available resources to help care for patient needs and refer to internal/external resources as appropriate, such as WellBe behavioral health protocols.
Collaborate with interdisciplinary team and participate in regularly scheduled team meetings.
Other tasks needed to accomplish team’s objectives/goals.
Telehealth – Nurse Practitioner Requirements
Minimum of Master's Degree in Nursing with focus on Family, Adult, Geriatric, or Adult‑Geriatric Nurse Practitioner from an accredited program.
5 years of progressive care experience required; experience in value‑based care preferred.
1+ year delivering clinical care via video‑enabled telemedicine.
Prior healthcare experience in medical assistant, nursing assistant, EMT/paramedic, lab assistant, registered nurse, or similar roles preferred.
Active, unencumbered Nurse Practitioner license in the state of the job posting and must reside in the state.
Current board certification (ANCC or AANP) required.
BLS certified and an active NPI# as an NP.
CPR certification required.
Active, unencumbered DEA licensure or eligibility to obtain required.
Experience in value‑based care, home care, primary care, hospice, palliative care, or geriatrics strongly preferred.
Valid driver’s license, car insurance, access to a car, and willingness to drive to patient homes.
Requires home visits to patients in various residential settings.
Requires proof of an active auto insurance policy.
Telehealth – Nurse Practitioner Skills and Abilities
Ability to conduct in‑home visits and have previous experience on a telehealth platform.
Proficient in Microsoft Office, G‑Suite, Electronic Health Records, scheduling software, and RingCentral.
Role model in teamwork, integrity, ethics, and productivity.
Experience diagnosing and treating ongoing, chronic, and urgent medical/psychological conditions of older adults.
Experience in panel management, primary care, geriatrics, internal medicine, and palliative care.
Experience in end‑of‑life communication.
Excellent verbal, written, presentation, and interpersonal communication skills.
Confident speaker to internal and external groups.
Video‑enabled telemedicine experience required.
Ideal location Central Time zone to align with Wisconsin market time zone.
Supervisory Responsibility This position will have supervisory responsibility.
Travel Requirements This position is primarily remote. If a candidate resides in‑market, clinical circumstances may require in‑person visits. If the candidate resides outside of market drive time, the position is 100% remote.
Work Conditions Ability to lift up to 30 lbs. Lifting or transferring patients may involve up to 50 lbs. as well as assisting with weights of more than 100 lbs. Ability to stand for extended periods. Ability to drive to patient locations. Requires fine motor skills and visual acuity.
