Home Care Registered Nurse (RN) – Rutland
VNA & Hospice of the Southwest Region
is looking to hire a full‑time
Home Care RN
in Rutland. You will provide quality home health and hospice care, supporting patients and families.

Work Schedule The role offers flexible hours. Clinicians create their own schedules to match client needs and maintain work‑life balance.
Day in the Life
Actively manage and assess patients’ treatment plans and needs.
Maintain accurate, up‑to‑date documentation of nursing care.
Ensure infection control standards and stay current on high‑tech skills (IV use, wound VAC, complex wound care).
Conduct timely visits and educate patients and caregivers.
Coordinate with physicians, providers, team members, and vendors to fulfill the care plan.
Assist with patient discharges or transfers and complete necessary paperwork.
Qualifications
Registered Nurse (RN) license.
Minimum 2 years of nursing experience (preferred).
Self‑directed, initiative‑driven, and collaborative within an agency.
Experience with electronic medical records.
Willingness to work a flexible schedule, including rotational weekends, holidays, and on‑call for hospice and home‑care patients.
Exceptional bedside manner, strong communication skills, compassionate demeanor.
Valid Vermont driver’s license and car insurance.
