and play a critical role in delivering compassionate and effective patient care within our dynamic healthcare program. This role will give you the opportunity to support and advance the health and wellbeing of our patients.
Base pay range $31.00/hr – $35.00/hr (actual pay based on skills and experience).
Schedule and Incentives
Full‑time positions: 7am‑3:30pm and 3:30pm‑12am.
Part‑time positions: 3:30pm‑12am and 11:30pm‑8am.
Rotating weekends.
Key Responsibilities
Patient Assessment and Care Planning
Assist in assessing patients’ health status and contribute to individualized care plans.
Direct Patient Care
Provide safe, effective nursing care for multiple patients within the facility’s program.
Implement patient education initiatives based on established teaching plans.
Nursing Scope of Practice
Perform delegated nursing tasks consistent with licensure and scope of practice.
Use problem‑solving approach to make informed clinical decisions.
Care Coordination and Monitoring
Oversee onsite and offsite patient scheduling in collaboration with care team.
Ensure follow‑up care, including telemedicine appointments and consult processes.
Quality Improvement and Collaboration
Participate in Quality Improvement initiatives and Patient Care Committee meetings.
Coordinate with Regional Medical Director, site medical providers, Nursing Director, and other leaders.
Documentation and Other Duties
Monitor CARES system and coordinate as needed.
Perform additional nursing duties to support patient outcomes and organizational goals.
Key Qualifications
Educational Background
Graduate from an accredited Licensed Practical Nursing program.
Licensure and Certification
Current licensure as an LPN in the state of employment.
Active CPR certification.
Skills and Knowledge
Familiarity with state laws and regulations governing LPN practice.
Successful completion of initial and annual clinical competencies.
Additional Requirements
Ability to pass background check, security clearance, and drug screening.
