About the Role Case Management at Epilepsy Foundation of Greater Chicago focuses on general epilepsy support through referrals to supportive resources, internally and externally of the organization, for those living with epilepsy. Internal services offered throughout Epilepsy Foundation of Greater Chicago generally comprise an average of 25 % of caseload time focus, and external/community resource referrals generally comprise and an average of 75 % of caseload focus. Case Managers can expect to carry caseloads of approximately 125‑140 members.
Responsibilities include both administrative and interpersonal, involving direct member support, coordination of services, and advocacy throughout internal and external referral.
General Duties
Schedule and complete member intake appointments, including gathering essential and documenting essential information, performing assessments and maintaining regular follow‑up appointments based on each individual case’s needs. Follow‑up frequency can vary. This includes completing all related intake/annual documentation and utilizing Salesforce to run reports and maintain a current member database.
Perform assessment of member’s social history, medical history, needs, strengths and preferences and other appropriate information prior to providing case management.
Formulate Individualized Service Plans including case closures; make resource referrals; and produce required documentation of services provided.
Provide education/information about epilepsy and related topics, both to assigned members and to others in the community who request it. Education and requests can be delivered via phone, virtual or face‑to‑face platforms during member follow‑up.
Refer/link members to EFGC internal programs and external service providers, including collaboration with internal and external resources. Support staff in moderating member programming as needed.
Maintain a significant member caseload that requires ability to work independently with strong organizational skills, including but not limited to providing varying levels of support, from scheduling appointments with individual members via phone, virtual or face‑to‑face platforms to discuss needs and provide internal and external support in areas of need.
Maintain accurate and comprehensive records of members’ interactions, assessments, and interventions through EFGC’s recording process; such as Salesforce, tracking sheets, calendars.
Provide crisis intervention support, conduct risk assessment and services to members, as needed.
Attend and participate in all required staff meetings, contribute to improving department processes, staff development activities, including internal case presentations and attending outside conferences/learning opportunities. Share information gathered at outside seminars with other staff. Work collaboratively with other team members and external agencies to ensure coordinated care.
Ability to travel on occasion, evening, or weekend work to assist program development events.
Other duties as assigned.
Qualifications
Required:
Bachelor's Degree in related fields such as Social Work, Psychology, and/or related social services field.
3 years minimum experience, 5 years preferred experience in Case Management in the social service field. Fluency in the Spanish language is preferred.
Required Skills
Must possess strong active listening, clear and concise verbal and written communication, interpersonal, exceptional organizational skills, problem solving, the ability to multitask and work independently, as well as demonstrate knowledge/skills needed to work successfully with persons with disabilities and under pressure and changing situations.
Familiarity with local and regional resources for public services; such as housing, healthcare, and other essential services.
Possess strong ability to advocate for member’s rights and needs.
Cultural competency and sensitivity to diverse cultural backgrounds and the ability to provide culturally appropriate services.

Pay range and compensation package
Salary Range: $54,000 to $64,000 commensurate with experience.
