Nurse Practitioner – Skilled Nursing Facility
Location: Orwell, OH
Schedule: Full-time, Monday–Friday
Compensation Base Salary: $100,000 – $150,000 per year

Position Overview Working collaboratively as a member of the medical staff, the APRN will play a critical role in managing the medical needs of residents within skilled nursing facilities. The APRN will build strong working relationships with administrators and staff throughout their designated building and serve as the primary point of contact for emergencies and urgent care visits. Ultra Health provides our APRNs with the training and tools to do more of what they love—working independently to see patients and treat complex medical conditions.
Responsibilities
Provide primary, urgent, and preventative care for residents in assigned skilled nursing facility; may also provide care to assisted living facility residents.
Verify admission orders and conduct initial assessments of residents, including physical exams, medication reviews, and depression screenings.
Collaborate with the facility medical director, attending physicians, and nursing staff to deliver the highest level of care.
Communicate with and counsel family members on treatment options when there are material changes in status.
Serve as primary point of contact for emergencies and urgent care visits.
Minimum Qualifications
Valid Nurse Practitioner License in Ohio.
DEA License.
National Nurse Practitioner certification.
A clinical background in adult, family, or geriatric specialties.
Vaccination against COVID‑19 required for all new hires unless the individual qualifies for a remote, religious, or medical exemption.
Preferred Qualifications
Experience in internal medicine, gerontology, or cardiology.
Excellent administrative, organizational, and verbal skills.
Ability to work independently.
Detail oriented.
Dependable and reliable.
Nursing experience in LTC/SNF a plus.
