Company Overview Residential Transitions, Inc. (RTI) is a Minnesota‑based organization dedicated to helping people improve their quality of life by empowering them with knowledge, resources, and skills while treating them with dignity and respect. RTI serves individuals across Minnesota who face mental illness, substance use challenges, and other disabilities, supporting independent living in the community, in their own homes, and in RTI’s residential settings.

Position Overview RTI is seeking a dedicated Waiver Case Manager for Dakota County. The role involves providing person‑centered case management services to individuals diagnosed with serious and persistent mental illness, substance use disorders, and co‑occurring medical conditions who qualify for the Community Access for Disability Inclusion (CADI) or Brain Injury (BI) Waiver Case Management services. In‑person training at the main office is required for the first 60 days.

Responsibilities

Manage and maintain a caseload of clients diagnosed with serious mental illness, substance use disorders, and co‑occurring medical conditions.

Visit clients at least once a month and twice annually per waiver‑specific rules.

Coordinate client intake and discharge processes within set parameters and timelines.

Collaborate with physicians, mental health providers, and other professionals to develop and update client plans.

Ensure financial authorizations and provider contracts are in place.

Document services in individual records to meet record‑keeping standards.

Access community and housing resources to match clients’ needs and preferences.

Provide crisis assessment and intervention to keep clients safe in home and community.

Required Qualifications and Experience

Qualified under Minnesota law as follows:

Graduate from an accredited four‑year college with a major in social work, psychology, sociology, or a closely related field.

OR graduate from an accredited four‑year college with a major in any field and one year of experience as a social worker, case manager, or care coordinator in a public or private social service agency.

Willingness to travel within the metro area and a current driver’s license; clean record; personal vehicle with insurance for client transport.

Must not be actively receiving services from Residential Transitions, Inc. or have received services within the last two years.

Preferred Qualifications and Experience

Master’s degree in behavioral science or a related field.

Experience providing direct services to adults with serious and persistent mental illness.

Experience with case management and commitment proceedings.

Compensation $50,000 – $55,000 per year

Physical and Mental Requirements

Walk, stand, sit, bend, climb stairs, and perform crisis prevention/intervention techniques.

Use hands for writing, typing, and data entry.

Lift up to 40 lbs and occasionally perform physical effort during duties.

Maintain vision and hearing required for reading charts, operating office equipment, using a computer, and driving a vehicle.
