location. Reporting to the Clinical Coordinator, the ideal candidate is flexible, compassionate, and professional. You will provide effective case management, clinical perspective, and service coordination for families in accordance with the Arizona Practice Model 12 Principles. Schedule:
Full-Time, Employed - Monday to Friday, 9am–6pm (No Weekends). Additional language differential pay is available.

Duties Include
Conduct the engagement process with families to identify natural/formal supports as outlined in the Arizona Model, Child and Family Team (CFT) process, and Regional Behavioral Health Authority (RBHA).
Facilitate CFT/Service Plan meetings according to the Arizona Model and RBHA guidelines to create individualized service plans.
Ensure families understand the behavioral health process, the role of CFT Facilitator, and facilitate family participation that links them to natural or community‑based supports.
Coordinate and schedule CFT/Service Plan meetings with natural, formal supports, and system stakeholders.
Submit appropriate referrals to clinical outpatient services within the agency and other behavioral health providers identified by the CFT.
Update and maintain electronic medical records on each case, including annual assessment updates, RBHA demographics, Child and adolescent service intensity instrument (CASII), and CFT/Service Plan.
Complete strength assessments and crisis/safety plans with families according to RBHA and the Arizona Department of Health Services (ADHS) guidelines.
Develop positive supportive relationships with consumers, families, stakeholders, and other Terros employees.
Complete Intake/Core Assessment as needed, according to RBHA and ADHS guidelines.
Position Summary: The Children’s Case Manager provides effective case management, clinical perspective, and service coordination for families in accordance with the Arizona Practice Model 12 Principles. Under agency policies and professional requirements, the role is a member of an interdisciplinary clinical team, managing clients, assisting with treatment coordination, and providing in‑office and in‑home family support.
Works under the direct supervision of the Children’s Clinical Manager.
