LTSS Service Coordinator—Clinician (Evansville/Mt. Vernon/Richland/Rockport)
Elevance Health is hiring an LTSS Service Coordinator—Clinician to provide field‑based assessment and care coordination for members in the Indiana region. The role reports to a supervising RN and involves evaluating member needs, coordinating services, and ensuring cost‑effective use of health benefits.
Benefits Sign‑on bonus: $2,500. Compensation range: $80,000–$122,800. Schedule: Monday‑Friday 8 am‑5 pm EST. Travel may be required for client sites.
Location Candidates must reside in Gibson, Perry, Posey, Spencer, Vanderburgh, or Warrick counties.
How You Will Make an Impact
Assist the RN in identifying high‑risk members.
Collect and provide clinical data for assessments and care planning.
Evaluate alternative levels of care or waiver programs with the RN.
Participate in coordination of care for members with chronic illnesses, comorbidities, or disabilities, ensuring efficient use of benefits.
Contribute to the development, monitoring, and revision of member care plans.
Minimum Requirements
Licensed LSW, LCSW, LMSW, or another non‑RN license in accordance with state law.
Minimum two years of experience in a service‑coordination or similar role with individuals who have chronic illnesses or disabilities.
Current, unrestricted state license.
Preferred Qualifications
MA or MS in Health or Nursing.
State‑specified certification, if applicable.
Willingness to travel to client sites as required.
Vaccination Requirement All new candidates for patient/member‑facing roles must be vaccinated against COVID‑19 and influenza. Offers may be rescinded for non‑vaccinated candidates unless an acceptable exemption is provided.
