Job Description Park Nicollet is looking to hire a Bereavement Coordinator to join our Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Position Summary The purpose of this position is to implement and maintain bereavement services including emotional, psychosocial, and spiritual support after the death of the patient. In addition, this position assists patients, families and caregivers in coping with issues related to grief, loss and adjustment. Education and support is offered to hospice staff, referral sources, and the community.

Work Schedule Monday – Friday 8:00am-4:30pm

Required Qualifications

Education, Experience or Equivalent Combination:

Master’s degree in social work, psychology, mental health counseling or a related field.

Two years’ professional experience with grief, loss and/or related issues.

Licensure/Registration/Certification:

Current MN Driver’s License and daily access to reliable, insured vehicle.

Knowledge, Skills, and Abilities:

Demonstrated experience in group process. Demonstrated ability to establish rapport quickly with people of all backgrounds.

Experience and expertise in establishing strong working relationships with interdisciplinary teams.

Excellent organizational and administrative skills.

Experience working with Outlook, Word, and Excel.

Preferred Qualifications

Education, Experience or Equivalent Combination:

Two years of hospice bereavement counseling experience.

Licensure/Registration/Certification:

Certification in Bereavement Counseling e.g. The Association for Death Education & Counseling (ADEC) or The American Academy of Grief Counseling

Essential Duties

(65%) – Provides pre‑ and post‑death counseling visits that include education, referrals to the community, resources and support per the patient/family/caregiver plan of care.

(15%) – Coordinates and conducts grief group sessions and educational programs under the direction of Leadership.

(5%) – Assures an initial assessment of risk level to determine the needs of the patient, family and other individuals focusing on the social, spiritual and cultural factors that may impact their ability to cope with the patient’s death.

(5%) – Incorporates information from the initial bereavement assessment into the patient/family/caregiver plan of care. Provides for ongoing assessment and updates to the plan of care before and after the patient’s death.

(5%) – Participates in interdisciplinary team meetings to assure appropriate bereavement coordination and care for patients, families and caregivers.

(5%) – Maintains accurate and timely documentation of all contacts with hospice patients, families and caregivers.
