13 Weeks Job Description: We are seeking a dedicated and compassionate Social Services Coordinator to join a team in a long term care facility in Seaford, DE. As a LTC Social Services Coordinator, you will be responsible for providing social services and support to individuals in need, coordinating community resources, and advocating for clients' rights and well-being. Responsibilities: Conduct assessments to determine clients' needs and develop individualized care plans Provide counseling and support to clients and their families Coordinate and collaborate with other healthcare professionals and community agencies Advocate for clients' rights and access to services Maintain accurate and up-to-date documentation of client interactions and progress Qualifications: Bachelor's degree in Social Work or related field 1 year of long term care experience State licensure or certification as required Previous experience in social services or a related field Strong communication and interpersonal skills Ability to work independently and as part of a team Compassionate and empathetic approach to client care If you are passionate about helping others and making a positive impact in the community, we would love to hear from you!
