Western Reserve Area Agency on Aging (WRAAA) is a private non‑profit corporation organized by the State of Ohio to serve as the planning, coordinating, and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain, and Medina Counties. WRAAA collaborates with local service providers and the Ohio Department of Aging to deliver home and community services and support to Ohioans, enabling them to age successfully in their homes and communities.

Position Description The MyCare Ohio Case Manager is responsible for the assessment and care management of long‑term clients ages 60 and older enrolled in waiver programs. The role requires a licensed social worker (LSW) and at least one year of experience working with individuals with physical or behavioral disabilities in home health care, medical social work, or behavioral health settings. This position offers a competitive salary and a mix of home and community‑based case management.

Qualifications

Current licensure as a social worker by the State of Ohio Counselor and Social Work Board.

At least one year of experience working with individuals with physical or a home health care, medical social work, or behavioral health environment.

Excellent verbal and written interviewing and assessment skills.

Basic knowledge of major chronic disabling illnesses for persons of all ages, disease processes, normal aging, and special knowledge in pediatrics and young adults.

Ability to effectively communicate with disabled and elderly populations and the general public, both in person and by phone.

Ability to communicate in a collaborative, effective manner with others and to maintain good working relationships.

Ability to comprehend written material related to essential job functions.

Ability to comprehend basic medical terminology.

Knowledge of local resources for older adults and persons with disabilities.

Ability to comprehend, recall, and apply facts related to essential job functions.

Ability to analyze, evaluate, and implement a reasonable course of action based on available information.

Attention to detail in client health and environmental safety assessments.

Physical ability to ambulate in private homes/access locations that may not be readily accessible.

Basic computer skills, including Microsoft Office (intermediate to advanced preferred).

Capacity to transport a laptop and case; able to lift/carry a minimum of twenty (20) pounds.

Must have a car available during working hours and be able to legally drive.
