We have an for APRN or PA to work in Skilled Nursing Facilities in the Waterford/New London area. This position offers a flexible work schedule, autonomy, and a pay for performance environment.
The Nurse Practitioner or Physician Assistant will travel to Skilled Nursing Facilities contracted with PACT, LLC.
Responsibilities
Provide medical care to a complex patient population in short-term rehab and long-term care settings.
Analyze patient’s medical history, medication allergies, physical condition, and examination to determine best care for acute and chronic medical conditions.
Order, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs.
Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate for assessed medical conditions.
Record in medical record the physical findings, formulated plans, and prognosis based on patient’s condition within appropriate time frame.
Coordinate with the nursing facility care planning team in the development of comprehensive care plans.
Communicate with patients and family members regarding admission, care plan, discharge needs, prognosis, and end-of-life care as needed.
Follow established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
Education and Experience APRN
Master’s or Doctorate prepared.
Certified nationally as a Nurse Practitioner through AANP or ANCC.
Valid Connecticut license as a Registered Nurse and Advanced Practice Registered Nurse, and certified as a Nurse Practitioner by an approved certifying body.
DEA and Connecticut controlled substance license.
Experience working with elderly patient population.
Experience working with EHR.
PA
State license.
