Social Worker - Case Management (Per Diem, Varied)

Compensation Range : $35.75 - $48.26 per hour based on applicable experience
Shift : PMs Hours per pay period: Varied Shift length: 10 hours Days off: Variable; weekends included
Position Summary The Social Worker is a member of the case management team that coordinates care and services for assigned patients during an episode of illness or treatment. The Social Worker works with the patient, family, physician, care team, and payors, utilizing advanced psychosocial skills to facilitate care coordination through assessment, planning, intervention, and evaluation. The Social Worker promotes and evaluates the effective utilization of resources, using current knowledge of community services and assuming a leadership role within the patient care team to achieve optimal clinical and resource outcomes.
Minimum Qualifications
Master’s degree in Social Work (completed within 2 years of hire) or Bachelor’s degree (for employees hired before August 2015)
Current CPR certification (within 3 months of hire)
Desired Qualifications
Experience in acute medical care and case management
Two years of medical social work or counseling in a medical setting
Skills & Knowledge
Knowledge of state and federal funding sources and community services
Understanding of third‑party reimbursement methodologies
Working knowledge of managed care requirements and hospital information systems
Assessment, planning, intervention, and evaluation of psychosocial aspects of patient care, including compliance and cultural issues
Strong crisis intervention skills
Critical thinking and analytical skills
Excellent communication and interpersonal skills, establishing rapport and professionalism
Ability to build positive relationships with physicians and external agencies
Proficiency with Microsoft Office, EPIC, and MIDAS
Commitment to maintaining strict confidentiality and fulfilling essential functions of the position
