Under the supervision of the Program Manager, CalAIM and general direction of the Vice President of Resident Services–Affordable Housing, the Case Coordinator (CC, CalAIM), supports the development, implementation, and expansion of CalAIM (California Advancing and Innovating Medi-Cal) Program-Community Supports. The Case Coordinator, CalAIM, ensures that programs and resources align with HumanGood’s goals and CalAIM regulations across several affordable housing communities.

This position is responsible for supporting the CalAIM Initiative in various areas, including case management, program implementation, partner relations, program outreach, client authorizations, and direct client services in collaboration with designated service providers. The position is responsible for supporting the implementation of CalAIM, with a focus on strengthening resident housing tenancy. Key responsibilities for the Case Coordinator include maintaining accurate case notes, monitoring resident progress, and ensuring timely documentation of services provided. The role also involves tracking relevant data and contributing to reports that support program compliance and resident outcomes. Additionally, the CC contributes to the strategic fulfillment of CalAIM objectives, working to enhance the health and independence of residents within the communities.

Ability to communicate in Spanish and/or Chinese is preferred.

Client Services and Support

Deliver comprehensive case management support and develop individualized care plans for qualified residents under the CalAIM- Housing Tenancy and Sustaining Services Program.

Maintain regular communication with residents to ensure continuity of care, track progress, and address any challenges they may face in accessing services.

Identify residents with high‑risk health needs or gaps in care and coordinate with clinical healthcare teams for comprehensive support.

Meet with clients regularly to identify client needs and make timely referrals to ECM and other Community Supports providers and services.

Document and review all services and encounters on the SpriteHealth platform.

Organize and securely store CalAIM‑related documents and files in OneDrive.

Actively participate in CalAIM service provider meetings.

Build and maintain strong relationships with service providers to ensure timely delivery of necessary services and program compliance.

Proactively identify and resolve key issues with service providers.

Develop a thorough understanding of SpriteHealth platform.

Program Implementation

Gain deeper understanding of resident populations at the designated CalAIM communities to enroll them into CalAIM programs and services in a timely manner.

Support the implementation of CalAIM Program by conducting outreach and education regarding the CalAIM program.

Support the development and implementation of HumanGood’s policies and procedures to ensure continuous compliance with program regulations.

Support the development and adoption of the program’s standard operating procedures to ensure high quality service delivery and resident outcomes.

Build a close collaborative working relationship with community housing administrators and resident services coordinators to effectively achieve CalAIM objectives and resident goals.

Coordinate cross‑functional strategies for optimum impact and scale.

Build and strengthen partnerships with HumanGood departments, local service providers, and the CalAIM provider network.

Track and maintain detailed case notes.

Deliver clear and concise oral and written reports on CalAIM services to designated HumanGood representatives and other stakeholders.

To be successful in this role, you would have:

Bi‑lingual: Ability to communicate in Spanish and/or Chinese is preferred.

Bachelor’s Degree in Gerontology, Social Work, Public Health, Healthcare Administration, or related field.

2+ years of experience serving older adults.

1+ year of experience in case management.

Experience working with vulnerable individuals is preferred, including individuals experiencing homelessness, at risk for avoidable hospital or Emergency Department utilization, with serious mental health, Substance Use Disorder needs, or at risk for long term care institutionalization.

Experience with administration of various assessment tools, creating, implementing and monitoring care plans for residents and care coordination with other providers.

Working knowledge of social, economic, and political issues relevant to low‑income communities.

Familiarity with trends in the field of healthcare, housing and social services.

Understanding of current federal, state and local laws, regulatory codes, ordinances, procedures relevant to social services programs, including those aimed at improving housing, education, employment, or socio‑economic status.

Principles and practices of performing needs assessments; program development, implementation, management and evaluation.

Conflict resolution and problem‑solving techniques.

Experience with community outreach, advocacy and public education.

Principles and practices of staff management, supervision and training.

Strong oral, written, presentation, interpersonal and diplomacy skills.

Location: This position is 100% onsite across (3) properties in the South San Francisco/Daly City region.

Compensation: $30–33/hr, depending on experience.
