Act as the primary point of contact for assigned patients across all enrolled Care Management Programs. Coordinate with support staff for tasks such as referrals, DME, labs, and diagnostic orders. Creation of care plans. Identification of Gaps in cares. Track patient progress for all programs (APCM, CHI, PIN, BHI) and update records in eClinicalWorks (eCW)/Ecares. Monitor adherence to care plans and ensure timely follow-ups. Proactively identify issues across programs and escalate unresolved or complex cases to the care manager. Communicate with patients and caregivers to provide updates on all programs. Responsibilities
Maintain accurate documentation of services provided, including time tracking for program billing. Collaborate with support staff to ensure smooth task execution. Participate in quality reviews and team meetings to share insights. Qualifications
