Overview Monday - Friday 8:00 AM – 4:30 PM Must have a WV Social Work License. Responsibilities
Oversee the Social Services Department operations in accordance with the National Association of Social Workers (NASW) Code of Ethics and all applicable federal, state, and local regulations and policies.
Foster a climate that supports patients/residents in maximizing their individuality, independence, and dignity.
Ensure each patient/resident receives a psychosocial assessment identifying their needs and strengths.
Identify and document patient/resident discharge goals at admission and assess progress toward those goals.
Educate staff on the role of Social Services and the psychosocial needs of patients/residents and their families, including issues related to aging and disability.
Supervise two or more Social Services staff, as appropriate.
Accountable for measuring outcomes, interventions, and departmental productivity.
Qualifications
Bachelor’s degree in social work or human services.
State licensure or certification required for long‑term care practice.
