Job Description Responsible and accountable for assessing the psychological, social, emotional, cultural, or financial needs that impact patients’ health and recovery. Provide professional social work services utilizing supportive counseling and clinical knowledge in assessing needs of patients and families during illness, crisis intervention, and post-discharge planning. Collaborate with members of the healthcare team in developing transition plans and addressing needs in a timely manner. Participate in program planning and development to meet customers’ needs within the healthcare system and community.

Shift Part-time, every Thursday and Friday 8 a.m. - 4:30 p.m. at the Barry Road location.
Requirements
Current Missouri LMSW
Prefer experience
Key Responsibilities
Completes individual and family therapy sessions with patients and their families.
Participates in daily treatment team meetings.
Provides psychoeducational group therapy sessions to patients.
Assists with discharge planning, making referrals, and assisting with transportation arrangements.

Job Requirements
Applicable Experience: Less than 1 year
Licensed Masters Social Worker (MO) – Missouri Division of Professional Registration
Master’s Degree
