Social Worker

UH Rainbow Babies & Children’s Hospital

A Brief Overview Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.
What You Will Do
Provide psychosocial assessment/intervention to assist patients/families (including clinically complex).
Provide age-specific care to patients/families in all age groups.
Develop, document and implement a comprehensive plan based on identified psychosocial needs and/or discharge needs.
Develop plans with attention to individual patient and family goals.
Provide linkage to community resources and UH network services to facilitate optimal care; and serve as liaison to community agencies.
Communicate/collaborate with the work team using a multidisciplinary approach.
Provide guidance and serve as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
Assist with recruitment, orientation, mentoring, and education of new staff and social work interns.
Additional Responsibilities
Perform other duties as assigned.
Comply with all policies and standards.
Refer to documentation provided by the department during orientation for specific duties and responsibilities.
Abide by all requirements to safely and securely maintain Protected Health Information (PHI) for patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
