Job Overview Fairview is looking for a Medication Scribe to join our team at the University of MN Medical Center West Bank Campus in Minneapolis, MN. The Medication Scribe is responsible for gathering and documenting patient medication information into the electronic health records system, as well as assisting with related administrative tasks to increase efficiency and productivity of the care team and satisfaction of patients.
Position Details
0.7 FTE (56 hours per pay period)
Evening shift (3 p‑11:30 p)
Every other weekend
Job Responsibilities
Prepare current patient‑specific medication lists in accordance with department processes and procedures, gathering patient medication and allergy information from patients, families, pharmacies, and other care facilities.
Clarify obvious drug problems in medication regimens.
Triage patients to provide an accurate medication list available for physician review, using all communication options available including e‑mail, fax and direct patient or provider contact.
Enter medications and allergies into patient profiles as appropriate.
Required Qualifications
Vocational School: Vocational/Technical
1–2 years of healthcare experience and/or equivalent educational experience.
2 years of healthcare experience.
Completion of a PTCB‑Recognized Medication History Education/Training Program within 6 months of hire.
Preferred Qualifications
Associate’s Degree in Nursing
2 years working as an LPN, CMA, CNA, TMA, or in a pharmacy environment.
At least 6 months experience conducting medication histories and/or similar patient‑focused communication.
Licensed Practical Nurse
Certified Medical Assistant
TMA or Certified Pharmacy Technician
