We are seeking an ambitious and mission-driven Remote Patient Monitoring (RPM) Coordinator to support and expand our growing care management team. This role will center on ensuring full compliance with RPM billing regulations, driving clinical outcomes through engaged patient relationships, and maximizing patient adherence and data utilization to ethically and effectively support our billing practices. RPM management tools include Blood Pressure Monitoring, Weight Monitoring, Pacemaker monitoring, Loop Monitoring, CGM monitoring, Peak Flow Monitoring, etc. This position will integrate CCM (Chronic Care Management), RPM (Remote Patient Monitoring), and RTM (Remote Therapeutic Monitoring) responsibilities, recognizing that the most impactful care comes from a cohesive, patient-centered approach. Responsibilities
Regulatory Compliance & Billing Integrity
- Ensure that all RPM, CCM, and RTM activities meet or exceed CMS guidelines and compliance standards. Maintain detailed documentation and encounter logs to support billable time and services rendered. Collaborate with billing and compliance teams to verify proper coding and documentation. Patient Advocacy & Clinical Support
- Serve as a primary point of contact for patients enrolled in RPM programs, ensuring understanding of their devices, expectations, and care plans. Monitor incoming device data, identify concerning trends, and elevate to providers as needed to support timely interventions. Educate patients on their conditions, medication adherence, lifestyle adjustments, and the importance of consistent device usage. Patient Engagement & Adherence Optimization
- Strategically manage and optimize patient interactions to ethically meet time thresholds for CPT billing codes (99453, 99454, 99457, 99458, 98975, 98976, 98977). Proactively follow up with patients who show signs of non-compliance or data drop-off to re-engage them in their care. Cross-Program Coordination (CCM/RPM/RTM)
- Identify opportunities to enroll patients across CCM, RPM, and RTM programs as clinically appropriate. Leverage shared tools, workflows, and communications to ensure streamlined and complementary delivery of services. Work collaboratively with NPs, MAs, and clinical teams to review care plans, prioritize patient outreach, and close care gaps. Program Growth & Innovation
- Support the implementation of new devices, services, and tools as the RPM/RTM program expands. Provide feedback on device performance, patient satisfaction, and workflow efficiency. Stay current on best practices and CMS updates related to remote monitoring services. Qualifications
Required: Clinical background (MA, RN, LPN, CMA, EMT or equivalent experience in care management). Knowledge of CMS regulations for RPM, CCM, and RTM billing and compliance. Exceptional documentation and organizational skills. Proficient in EHR and remote monitoring platforms (e.g., Tenovi, Withings, ResMed, React, etc.). Healthcare/Cardiology/Telemetry Experience Preferred. Preferred: Experience in remote care delivery or care management programs. Ability to interpret physiological data and triage accordingly. Strong interpersonal skills, with a confident but compassionate communication style. Previous experience as a Care Coordinator or in a similar position is preferred. Strong problem‑solving and organizational skills. Ability to work in fast‑paced situations and make sound decisions quickly. Excellent interpersonal skills and high level of compassion. Strong verbal and written communication skills. Ideal Candidate Traits
