Responsibilities
Work collaboratively with healthcare teams to assess and plan interventions that help patients cope with social, emotional, financial, and environmental challenges. Perform psychosocial assessments and provide interventions, including counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving, and coordinate family/team meetings. Document treatment recommendations, professional recommendations, care coordination interventions, and treatment plans. Engage patients and families to adhere to the treatment plan by reducing or eliminating barriers. Maintain a network of resources for consultation and connect patients with community resources. Supervise Social Work Interns and assist with training new caregivers. Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes. Minimum Qualifications
Master’s Degree in Social Work from a CSWE accredited program. Current state licensure as a Social Worker (LSW). Licensed Independent Social Worker (LISW) within five years of hire. At least two years of experience in a hospital and healthcare setting. Preferred Qualifications
Additional social work experience. Experience using Epic Electronic Health Records. Physical Requirements
Manual dexterity to operate office equipment. Ability to sit, stand, and walk to various locations throughout the facility to attend meetings. Normal or corrected vision. Clear verbal communication by phone. Personal Protective Equipment
Follow Standard Precautions and use personal protective equipment as required. Pay Range
Minimum Annual Salary: $57,510.00 Maximum Annual Salary: $87,697.50 The pay range displayed reflects the anticipated range for new hires. Actual compensation will be determined based on the candidate’s work history, experience, skill set, and education. The salary does not include shift differentials, overtime, or the value of the Cleveland Clinic’s benefits package (e.g., healthcare, dental, vision, retirement contributions).
