Responsibilities
Supervises the coordination of a wide variety of administrative, operational and technical activities to ensure safe and efficient Transfer Center operations.
Facilitates communication between sending and receiving facilities using telephone, computer software, and other communication tools.
Documents a record of the encounter.
Utilizes clinical expertise, critical thinking, and knowledge of unit‑specific admission and discharge criteria.
Assumes 24‑hour responsibility for all departmental business and operations.
Acts as Acting Manager in absentia, making administrative decisions, holding meetings, ensuring timekeeping and payroll procedures, and providing accountability.
Assists in mandatory reporting to local administrative leadership and network personnel.
Responsible for quality assurance and team member feedback.
Assists with the coordination of team meetings.
Participates in recruitment, onboarding, and separation processes.
Represents the Transfer Center on appropriate committees, task forces, and professional organizations in a professional manner.
Communicates regularly with peers, superiors, and subordinates about workflow, protocols, network updates, and new procedures.
Helps create and foster a respectful and inclusive team environment.
Is self‑motivated, independent, yet seeks guidance from senior leadership when needed.
Answers telephones in accordance with established policies and procedures.
Utilizes a professional tone and demeanor in all communication.
Performs all tasks in accordance with Clinical Coordinator RN and Transfer Center Agent job descriptions.
Hands‑on with patients across all ages: neonatal to geriatric.
Must be able to sit at a computer with multiple monitors while speaking on a telephone for extended periods.
Other duties and/or projects as assigned.
Qualifications
Current and valid New Jersey Nursing license is required.
Basic Life Support/CPR certification by the American Heart Association within 60 days of hire.
Bachelor of Science in Nursing (BSN) from an accredited school; MSN preferred.
Minimum five years of clinical experience in an acute care setting.
Transfer Center experience required.
Critical care and emergency medical services experience preferred.
Ability to learn quickly, adapt to changing patient needs, demonstrate accountability, strong teamwork, dedication to ongoing education, and a passion for delivering highest quality patient care.
Compensation Starting at $124,571.20 annually. HMH is committed to pay equity and transparency. The posted rate is a good‑faith estimate of the minimum base pay and does not reflect the full value of the competitive total rewards package.
