Case Manager, Days
Rosecrance Behavioral Health – Ware Center, Rockford, IL (On‑site). Full‑time, 40‑hour work week.
We provide compassionate, evidence‑based behavioral health services. Your role will help individuals and families recover from substance use and mental health challenges by reducing hospitalizations and supporting community functioning.
Position Summary
Provide mental health rehabilitation services and case management to adults and families.
Decrease hospitalization and crisis episodes while increasing community functioning.
Maintain ethical, legal, and moral standards in a safe therapeutic environment.
Participate as a team member to deliver services and facilitate recovery in daily living situations.
Perform case management, crisis intervention, skills training, counseling, medication monitoring, quality improvement, and related duties for seriously mentally ill clients.
Qualifications
Minimum Bachelor’s Degree in Psychology, Counseling, Social Work, Addiction Studies, or related field; or equivalent experience and training.
Qualified as a MHP or QMHP per Illinois Department of Mental Health Rule 132.
Valid driver’s license and own transportation (designated positions).
Excellent written and oral communication; computer proficiency required.
Proficient in navigating electronic health records and completing documentation within prescribed time frames.
Skilled in individual, family, and group counseling; ability to present lectures for clients, parents, staff, or community resources.
Self‑starter capable of working independently and within a team.
Belief in the mission and vision of Rosecrance.
Key Responsibilities
Work prescribed hours and additional hours as needed.
Motivate and problem‑solve with staff regarding effective clinical practices.
Adapt communication to the client and family language and style.
Complete all necessary documentation within appropriate timelines.
Undertake primary case management responsibilities for designated clients.
Conduct individual, group, and family counseling and educational sessions.
Maintain client service records consistent with regulations.
Coordinate services from a variety of community resources.
Utilize evidence‑based practices and service modalities.
Present workshops, lectures, and training to clients, families, staff, and community groups.
Be familiar with court proceedings and available for court appearances on behalf of clients.
Serve as a member of the Clinical Team and participate in all meetings and activities.
Exercise confidentiality per professional code of conduct and legal frameworks.
Deliver exceptional customer service consistently to every client.
Represent the agency positively and serve as a role model.
Assume other responsibilities as delegated by management.
Schedule & Work Mode
Full‑time; 8‑hour shifts; 40‑hour work week
Monday to Friday, 8:30 am to 5:00 pm
On‑site at Rosecrance, Ware Center, Rockford, IL
Compensation & Rewards
Base Pay: Starting at $46,800 / year (adjusted for education, experience, credentials)
Professional Certification bonus (e.g., CADC, CRSS, or CPS): +$0.72 /hour ($1,500 / year)
Sign‑on bonus: $3,000 for non‑licensed; $4,000 for 1st or 2nd‑level license
