Wellness Coach (Case Manager)

$50,000 – $55,000 per year Full‑time Flexible schedules Position Overview
We are seeking Wellness Coaches who are passionate about helping others and are ready to serve as case managers. In this role you will assist consumers with accessing needed community services—medical, psychological, substance abuse, vocational, food, clothing, and more—through the development of a referral network. This opportunity is ideal for those looking to build a career in the not‑for‑profit social services arena. With a commitment to helping others you can progress into positions such as Vocational Specialist, Clinical Coordinator, or Program Director. Responsibilities
Engage individuals with mental illness and help them meet the demands of community living, maintain housing, and develop necessary skills. Link individuals to services that support recovery, independence, and self‑determination. Conduct group meetings on wellness management. Complete service plan reviews for all residents. Assess, correct, and report on apartment and personal living space conditions as assigned. Provide case management for residents, help them set rehabilitation and housing support goals, and collaborate on developing a Housing Support Plan. Qualifications
Bachelor’s Degree in Human Services or related field required. Two years’ experience working with individuals with mental illness, substance use issues, and/or a history of homelessness preferred. Experience in scattered site settings strongly preferred. Experience in group work, tenant organization, case management, referral, and/or community outreach strongly preferred. Experience or training in peer counseling or advocacy preferred. Working knowledge of Microsoft Word, Excel, PowerPoint, and Windows operating system. Bilingual English/Spanish preferred but not required. Company Overview
