Position Overview: We are seeking a licensed Advanced Practice Provider to join our Chronic Care Management program in a full time, fully remote capacity. This role focuses exclusively on non face to face chronic care management services for an established patient population. The provider will manage patients with multiple chronic conditions, ensuring continuity of care, care coordination, and proactive patient engagement in compliance with CMS CCM guidelines.
Candidates must hold an active and unrestricted Washington State license and be comfortable working independently in a remote environment while meeting daily productivity expectations.
Key Responsibilities: Provide chronic care management services to patients with two or more qualifying chronic conditions Complete a minimum of 20 CCM encounters per workday Conduct non face to face patient interactions including phone calls, care plan reviews, medication reconciliation, and follow up coordination Develop, update, and manage comprehensive electronic care plans Coordinate care with primary care providers, specialists, facilities, and ancillary services as needed Document all patient interactions accurately and timely within the electronic medical record Ensure all CCM activities meet CMS, payer, and organizational compliance standards Identify care gaps and escalate clinical concerns appropriately Participate in quality improvement initiatives related to chronic disease management Maintain productivity, quality, and documentation benchmarks
Required Qualifications: Active and unrestricted Physician Assistant license in Washington State National board certification as an PA Minimum of one year of clinical experience managing chronic conditions preferred Strong knowledge of chronic disease management and care coordination principles Comfort with remote work and telephonic patient engagement Excellent clinical documentation and organizational skills Ability to meet daily CCM productivity requirements consistently Strong communication skills and patient centered approach
Preferred Qualifications: Previous chronic care management or care coordination experience Experience working with Medicare and CMS CCM programs Prior remote or telehealth experience Familiarity with electronic medical records and CCM workflows
