is seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team. In this role, you’ll provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. This position is ideal for professionals who want to make a direct impact on the lives of older adults—helping them feel heard, supported, and emotionally connected within their environment. Key Responsibilities
Visit designated facilitiesin accordance with the schedule set with your supervisor.
Conduct assessmentsof residents to identify emotional, cognitive, and psychosocial needs.
Implement short-term, solution-focused therapyand supportive interventions to address immediate concerns.
Document visit notes and maintain detailed recordsof interactions and interventions after each visit.
Collaborate with psychiatric professionalsto ensure comprehensive care, including participation in amonthly meeting with the psychiatric NP.
Qualifications
Associates, Bachelor’s or Master’s degree inSocial Work, Psychology, Counseling, or another mental health–related field.
Understanding ofgeriatric mental health,crisis management, andpsychosocial supportin long-term care settings.
Strongcommunication,documentation, andrelationship-buildingskills.
Education: Associate Degree
Experience: 1 year
