Are you a Registered Adult Nurse with a passion for providing specialist continence and urology care? Join our dynamic Team, dedicated to delivering high-quality services across North East Essex.
Our service operates between 0800 - 1800, 7 days a week, providing essential care to patients in their homes and in nurse-led clinics, aiming to restore maximum independence and promote self-management in daily living.
As a vital member you will manage a caseload of patients with continence and urology needs, working closely with GPs, Integrated Care Teams (ICTs), and other statutory and voluntary agencies. Assess, treat, and review adults with continence needs and/or bladder and bowel dysfunction, ensuring individualised care plans are agreed.
During Nurse-Led Clinics you will work within nurse-led clinics and patients' homes, accepting referrals from healthcare professionals and associated agencies.
You will plan and implement care programs for the specialist management of continence problems, monitoring their effectiveness and adjusting as needed.

Main duties of the job
Asses, treat and review adults with continence needs and/or bladder and bowel dysfunction.
Prescribe medicines within a defined clinical area according to the scope of practise and professional competence of the individual.
Ensure that patient care plans are robust, individualised and are agreed with the patient and/or carers/ family members as appropriate.
Manage a caseload of adult patients in North East Essex who require continence and urology nursing care.
Work within nurse-led clinics and patients' homes, accepting referrals from other healthcare professionals and associated agencies.
Plan and implement a programme/plan of care for the specialist management of adults with continence problems and to monitor its effectiveness.
Provide counselling, psychological support as appropriate to adults and their carers/family members in response to their identified continence needs.
Assess and implement plan of care for complex catheters.
