Outreach Counselor
to join our team.
Outreach Counselor Key Responsibilities:
Conduct outreach to engage high-risk youth and their families in services.
Meet with parents/guardians to support youth engagement.
Provide counseling, employment, educational, and life skills training.
Coordinate with YOS services to connect youth and families to resources.
Offer transportation to youth as needed.
Build relationships with referral sources that are beneficial to clients.
Make referrals to appropriate internal and external programs, ensuring timely follow-ups.
Develop and implement service plans tailored to youth needs.
Maintain accurate documentation in electronic records.
Participate in team meetings, supervision, and collaborative agency programs.
Outreach Counselor Qualifications:
Bachelor’s degree in Social Work, Counseling, or Psychology (required).
Experience working with at-risk youth and families.
Familiarity with outreach programs.
Valid driver’s license and reliable transportation with proof of insurance.
