00875053## LocationNew York, New York## ## Shift:Day Flex (United States of America)## ## Description:**Support Our Communities**The Targeted Outreach program, located within NewYork-Presbyterian’s Division of Community and Population Health, promotes health and prevents disease through early detection and intervention. Wecollaborate with trusted community-based organizations, connecting community members with services, resources, and health education to address health and social needs. The Targeted Outreach team is launching new ways of working and deepening our presence in key NYC neighborhoods in 2026, making this an exciting time to join for a strong public health practitioner and creative self-starter.Specifically, we aim to improve behavioral and cardiovascular health among unhoused community members, new New Yorkers, and faith-based communities. Our initial focus will be on Crown Heights and University Heights, with future growth planned for NYP’s Community Service Plan priority neighborhoods in NYC and Westchester County (currently University Heights, Crown Heights, Washington Heights and Inwood, Corona, the Lower East Side, Peekskill, and Mount Vernon). Our interventions include pop-up clinics, screening events, wellness programming, and Mental Health First Aid training.

**Program Lead – Targeted Outreach

- Weill Cornell**Reporting to the Program Director, the Program Lead oversees initiatives within the Division of Community and Population Health’s Targeted Outreach program. The Program Lead coordinates closely with the Program Director and Medical Lead to operationalize, evaluate, and report on Targeted Outreach programs. They are responsible for managing administrative and programmatic responsibilities, including day-to-day operations, relationship building with community and NYP stakeholders, evaluation design, data collection, and reporting. Additionally, this role will work collaboratively with colleagues across the NYP enterprise and research partners at affiliated academic medical centers. The position will require travel to our priority neighborhoods for communityevents. Frequent evening and weekend hours are required. **Preferred Criteria*** Master’s degree* Familiarity with Crown Heights and/or University Heights* Experience working with marginalized communities or as a community organizer* Experience with mixed methods research design and implementation* Familiarity with behavioral or cardiovascular health public health interventions* Bilingual in Spanish**Required Criteria*** Bachelor’s degree* Minimum of three (3) years ofexperience* Experience in program development, implementation and evaluation* Experience working in a community setting* Excellent communication, analytical and interpersonal skills and high degree of flexibilityAbility to function independently with minimal supervisionJoin a healthcare system where employee engagement is at an all-time high.
