Job Description – Oncology Nurse Navigator – Lung, Head and Neck (906608)

Job Summary This position will assist patients, families, and caregivers to overcome healthcare barriers (social, financial, or physical). The navigator conducts ongoing assessments, facilitates referrals to support services, and uses evidence‑based nursing processes to provide education and resources that support informed decision‑making for patients and families.

Work Schedule Monday – Friday, 8:00 am – 5:00 pm

Qualifications

Associate’s Degree and graduation from an approved/accredited nursing program (Texas BON 217.1)

3 years oncology nursing experience

Registered Nurse (RN) license and Basic Life Support (BLS) certification accredited by the American Heart Association (AHA) or American Red Cross (ARC)

Preferred

Bachelor’s Degree – Bachelor of Science in Nursing

Oncology certifications: OCN, CPHON, CBCN, BMTCN

Job Duties

Assess barriers to care across the continuum and address patient, caregiver, or family needs.

Serve as the primary point of contact for providers, patients, and caregivers.

Advocate for patients and caregivers throughout the healthcare system.

Develop and deliver educational programs, resources, and tools to meet identified needs.

Provide referrals to internal and external resources based on assessment findings.

Facilitate shared decision‑making, ensuring full understanding of treatment intent, options, and outcomes.

Promote advance care planning and help patients articulate goals of care.

Ensure clear, concise communication among providers, departments, and patients.

Coordinate multidisciplinary planning and care conferences as needed.

Participate in outreach projects to close patient care gaps.

Perform other duties as assigned.
