We are seeking a compassionate, skilled and experienced Nurse Practitioner to join our community team to help deliver outstanding, person-centred care to patients in their own homes and community settings.

This is a key clinical role for a Nurse Practitioner with extensive specialist knowledge in palliative care and a commitment to ongoing clinical involvement and service development within this field. You will have or be working toward a palliative care degree or equivalent Level 3 Health Degree and/or working towards being a prescriber.

With your professional and friendly approach, will provide specialist assessment, advice, and nursing care to patients with complex palliative and end-of-life care needs. You will also establish and maintain supportive relationships with GPs, wider nursing teams and other partners to ensure the highest quality of care.

As a team player and natural leader, you will contribute to multidisciplinary team meetings to review and manage patient treatment plans and will act as a role model, sharing specialist skills and supporting the development of other practitioners.

You will manage referrals efficiently and ensure appropriate deployment of skilled staff to meet patient needs and on a rotational basis you will provide a triage service.

Leading by example you will also play a supportive role in delivering specialist palliative care education, promoting evidence-based practice and contributing to service development.

Main duties of the job Provide specialist advice, support, and nursing care to patients whilst establishing supportive relationships and presence with GPs and other community teams within an identified geographical patch (delivered within community and outpatient clinic settings).

Demonstrate palliative assessment skills, and will be able to support, illustrate, and disseminate such skills to other practitioners while benefiting patient care.

With support from the Palliative Care Community Consultant, will assess and support patients who are on our virtual ward and are experiencing complex symptoms or a crisis in their illness.

Attend daily/weekly Multidisciplinary meetings to review and manage patient treatment plans.

Work as part of a multidisciplinary team and other care providers, demonstrating autonomy within a geographical area.

Take a supportive role in the provision of specialist palliative care education, promoting evidence-based practice.

To provide a triage service on a rotational basis, receiving incoming calls, taking messages, and responding accordingly.

To manage referrals promptly through robust assessment and prioritisation processes and deploy appropriately experienced and skilled staff to meet the needs of those referred patients.

To ensure that there is efficient and effective utilisation of resources human and material to deliver optimum quality care to their client group.

To work flexibly to meet the needs of the Hospice at Home service.

Job responsibilities Scope of Practice/ Main Duties

Undertake a Practitioner role in palliative care through partnership with health care professionals and other agencies throughout Shropshire, Telford and Wrekin and Powys. Providing specialist advice and support on all aspects of clinical palliative care.

Work autonomously using evidence-based skills and translating expert knowledge directly into individualized, holistic therapeutic patient care in the full range of settings across primary care.

Develop clinically effective and therapeutic working relationships with patients and relatives using persuasive, negotiating, and motivating skills to communicate complex, contentious, and sensitive information. For example, end‑of‑life issues and difficult‑to‑accept conditions, taking into account barriers to understanding and challenging communication.

Demonstrates highly developed communication skills to share sensitive and complex concepts with other professionals and relevant organisations about a full range of palliative care issues.

To identify where complex grief patterns are present, facilitating referral to specialist bereavement support services.

To have in‑depth specialist knowledge and clinical expertise in palliative care, underpinned by accessing relevant training and updates.

Make clinical judgements which require analysis, interpretation, and comparison of a range of options, therefore recommending appropriate evidence‑based diagnostic and therapeutic interventions to patients and health care professionals. Refer to nursing and medical professionals, and appropriate statutory and voluntary agencies.

Working with the Community Services Manager to continually improve palliative care services.

Maintain reflective practice through individual clinical supervision and peer review.

Provide expert clinical support in the field of palliative care to inspire colleagues to improve standards and practice.

To attend and participate in team meetings and represent Severn Hospice when appropriate locally and nationally.

Record and submit accurate and timely data, as required.

Receive, manage, and triage referrals, maintaining good channels of communication with referring teams with daily updates on care given, outcomes, and continuation / withdrawal of service.

Work to ensure equity both in terms of access and service to patients who meet the criteria for care.

Education, Professional Development and Training

To maintain and acquire competencies and skills for self‑development.

To participate in the supervision, training, and effective mentorship of junior staff and Nursing Assistants.

To participate in informal and formal training sessions, deliver induction programmes for new staff, and provide relevant feedback to all members of the team.

Identify personal development and educational requirements through yearly review/appraisal.

Support the design and delivery of palliative care education to internal and external delegates.

Provide educational placements for healthcare professionals, sharing knowledge and advice.

Research and Audit

In conjunction with the Community Services Manager and Leads, support the identification of clinical audit requirements, collecting and collating data, and presenting findings.

To review and evaluate palliative care‑related research continually to maintain best and evidence‑based practice.

Supplementary Information and Requirements

All new Hospice employees are on a 6 month probationary period.

A DBS Disclosure will be requested in the event of an individual being offered the post.
