The Trinity Center atLuther Park is 120 bed facility with 60 CCDI beds with an overall carephilosophy of serving those in the dementia disease process. JOBSUMMARY:
The Infection Preventionist and Wound Nurse will lead and implement the infection prevention and controlprogram to control the spread of infections and/or outbreaks, support the organization'squality improvement program and structure and provide education to staff,residents and visitors as needed.
ESSENTIALFUNCTIONS:
Develop and implement an ongoinginfection prevention and control program to prevent, recognize and control theonset and spread of infections to provide a safe, sanitary, and comfortableenvironment.
Establish facility-wide systems forthe prevention, identification, reporting, investigation, and control ofinfections and communicable diseases of residents, staff, and visitors.
Develop and implement writtenpolicies and procedures in accordance with current standards of practice and recognizedguidelines for infection prevention and control.
Oversee resident care activitiesthat increase the risk of infection (i.e. use and care of urinary catheters,wound care, incontinence care, skin care, point-of-care blood testing andmedication injections).
Complete infection control auditsfor new hires and other employees (Hand hygiene, PPE, Glucometer use,peri-care, catheter cares)
Complete weekly NHSN reporting.
Update the infection control module.
Complete rounds and ensure residentson EBP and isolation have signage and equipment in place.
Review and/or revise the facility's infection prevention andcontrol program, its standards, policies, and procedures annually.
Ensure public health is notified of reportable diseases.
Remain current on new developments related to infection preventionand control through in-services, professional journals, laws and regulations.
Coordinate and complete monthly infection controlsurveillance rounds in various departments (Nursing, dietary, housekeeping,Laundry and maintenance) and bring attention to various departments' areas of concern.
Attend daily clinical huddles.
Participate in theon-call rotation as the manager on duty.
Wound Nurse
- Assess any new wounds to determine whetherthey are pressure/non pressure upon notification by the nurse.
- Notify the wound doctor of any new wounds andobtain consent so that the wound doctor can follow up with the resident.
- Complete wound rounds with or without wound doctor when the wound doctor is not available and completeN-Adv skin check assessments weekly.
- Complete and update care plans for all woundsfollowed by the wound nurse and wound doctor.
- Ensure all interventions are in place for allresidents with pressure wounds (Pressure relieving mattress and cushion,supplements and any other support equipment).
- Notify Resident and resident POA of woundprogress and interventions in place.

Minimum Job Requirements::
Must be a LicensedRegistered Nurse who currently possesses a valid license in good standing inthe state of Iowa.
Minimum of 1-3 years RN experience, preferably in a long-term care setting.
Previous management or supervisor experience preferred.
Previous infection prevention experience preferred.
Complete CMS/CDC Nursing Home Infection Preventionist Training Course withing 6 months of employment.
Knowledge of State and Federal regulations.
BLS certification required.
Must be able to relatepositively and favorably to residents, families and government regulators andto work cooperatively with other associates at all levels.
A post-offer employment physical,drug screen and background check are required.
