* Manage and support a multidisciplinary clinical team, including hiring, training, scheduling, and performance evaluation.
* Oversee individualized care plans, ensuring timely and effective interventions in coordination with physicians and healthcare partners.
* Ensure compliance with Medicare/Medicaid regulations, state laws, and agency standards, including Joint Commission requirements.
* Monitor clinical outcomes, conduct audits, and implement quality improvement initiatives for enhanced patient care.
Candidate requirements
* Bachelor's degree in Nursing (BSN) required
* Active RN license in the state of Arizona.
* Minimum of 2 years of clinical experience in home health or a related field, with prior supervisory or management experience.
* Basic Life Support (BLS) certification required.
