Position Purpose
The Clinical Liaison leverages strong interpersonal skills, clinical expertise, and collaborative energy to build and maintain relationships with referral sources for Schwab Rehabilitation Hospital. This role conducts timely on-site and remote clinical assessments to determine patient appropriateness for acute rehabilitation, coordinates admissions, and supports achievement of inpatient census goals.
Key Job Activities
Referral Development & Relationship Management
* Establishes and maintains strong referral relationships through face-to-face engagement with discharge planners, social workers, case managers, nurses, therapists, and physicians.
* Actively identifies and develops new referral sources by scheduling a designated number of quality appointments daily and maintaining consistent weekly outreach.
* Provides education to patients, families, and referral sources regarding acute rehabilitation services.
* Coordinates and schedules informational presentations for referral partners.
Clinical Assessment & Admission Coordination
* Conducts on-site, bedside, telephonic, and medical record pre-admission screenings to assess patient suitability for rehabilitation.
* Provides referral sources and families with a tentative 24-hour admission determination.
* Completes pre-admission screening documentation accurately and timely, ensuring physician review and approval prior to admission.
* Coordinates smooth transitions from referring facilities to Schwab Rehabilitation Hospital, ensuring medical stability and receipt of complete medical records.
* Provides recommendations for appropriate levels of care for patients denied admission.
Collaboration & Performance
* Collaborates closely with the Admitting Department and Access Manager to ensure timely admissions and exceptional customer service.
* Works as a dynamic team member with other Clinical Liaisons to achieve inpatient census and volume goals.
* Demonstrates strong multitasking, time management, and clinical decision-making abilities.
* Maintains a high level of responsiveness and professionalism with referral sources.
Quality Improvement & Customer Service
* Promotes patient and family satisfaction and participates in departmental quality improvement initiatives.
* Identifies operational challenges and contributes to solution development.
* Demonstrates and encourages excellence in customer service, teamwork, discretion, and confidentiality.
Additional Responsibilities
* Performs other duties as assigned in support of departmental and organizational goals.
Education & Work Experience
* Degree in Nursing, Social Work, Physical Therapy, Occupational Therapy, Athletic Training, or Speech Therapy required.
* Experience working autonomously required.
* Prior experience as a Clinical Liaison and/or in post-acute care preferred.
Knowledge & Skills
* Strong clinical assessment and communication skills.
* Knowledge of insurance, managed care, and admission criteria for acute and sub-acute rehabilitation.
* Ability to manage a highly variable workload and multiple priorities.
* Experience with EPIC and MEDITECH software systems preferred.
Certifications & Licensure
* Active Illinois professional licensure required.
* Valid driver's license with no restrictions required.
