Grievance Procedure

Disciplinary Consequences

About the Organization

The Little Foundation was established to provide home and community-based services for individuals with developmental disabilities in the greater Phoenix area.

Currently, The Little Foundation offers the following Home and Community-Based Services (HCBS) to eligible members through the Arizona Department of Economic Security, Division of Developmental Disabilities (DES/DDD):

- Respite (RSP): Temporary care provided to give primary caregivers a break.

- Attendant Care (ATC): Assistance with daily living activities to help individuals maintain their independence.

- Habilitation Hourly (HAH): Individualized support to develop and enhance skills and abilities.

- Habilitation- Group Home (HAB): Structured support in a group home setting to promote skill development and independence.

- Room and Board- Group Home (RRB): Provision of living accommodation and support in a group home setting.

Description

Direct Care Worker (DCW) Job Description and Expectations

The Direct Care Worker (DCW) ensures that the member's schedules are followed and that documentation is complete and accurate. The DCS is the frontline worker of the group home and completes the required tasks to ensure we meet the wants/needs of the members. The DCS's role is critical to The Little Foundation's goal of Making the Impossible Possible.

Roles and Responsibilities:

The Direct Care Worker position is paid an hourly wage dependent on experience and knowledge. To qualify for this role, you should have a high school diploma or equivalent, preferably some experience in caregiving or a related field.

* Assist members with daily activities such as personal care, meal prep/planning, and household chores.

* Provide companionship and support with social engagement and recreational activities.

* Maintain a safe and structured environment tailored to the individual's needs.

* Support the development of life skills and encourage independence.

* Follow care plans and communicate effectively with families and care teams.

* Ensure respect for personal preferences, routines, and boundaries.
