Community Health Nurse
As a Community Health Nurse, you will provide nursing interventions, patient education, and care management to a variety of clients in our community, promoting good health and one‑on‑one patient care in the comfort of their own homes.
Full‑time | Monroe Ave | 40 hours per week, day shift.

Two (2) years experience in home care preferred

At least one year of nursing experience
Responsibilities
Provide comprehensive assessments of the bio‑psycho‑social needs of acutely ill clients in their homes, perform skilled nursing care and prescribed treatments based on MD orders, and provide client and family education in accordance with the assessment and plan of care.
Develop and document individualized care plans customized for each patient’s unique needs, with support from the interdisciplinary health team as needed, and maintain effective communication to convey patient health status, treatment plans, and progress.
Maintain effective communication to convey patient health status, treatment plans, and progress; communicate with office and supervisory staff regularly; coordinate client services and/or referrals efficiently and timely.
Accurately and completely document client care; document and implement physician orders per D.O.H. and program standards.
Adhere to required department and system protocols, regulations (local, state, federal) and education requirements.
Education AS: Nursing (Required)
Licenses / Certifications
BLS – Basic Life Support – American Heart Association (AHA)
RN – Registered Nurse – New York State Education Department (NYSED)
Physical Requirements M – Medium Work – Exerting 20 to 50 pounds of force occasionally, or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; requires frequent walking, standing or squatting.
Pay Range $75,733.00 – $101,014.00 per year.
