Physician / Medical Director – Skilled Nursing Facilities / Nursing Homes

This role offers a strong work/life balance with flexible full‑time or part‑time hours. You will care for an adult geriatric population and collaborate with clinicians who are experts in their field.
The position requires traveling to 1‑2 nursing homes per day around Pittsburgh/New Castle, PA. Focus is on chronic disease management and palliative care; training is available for palliative care. Nurse practitioners provide support for initial assessments and documentation.
Schedule
Full‑Time: Monday – Friday, 8 a.m. – 4:30 p.m. (with flexibility)
No weekends (unless preferred)
Part‑Time: 16–32 hours per week
Ability to see patients in the morning/early afternoon and finish the day at home
Compensation
Base salary: $240,000 – $280,000 /yr (dependent on experience)
Bonus potential: $10,000–$12,000 per quarter (≈ $40,000–$50,000 annually)
Paid time off: 4 weeks PTO
401(k) with company match (100% of contributions on the first 3% and 50% on the next 2%)
CME allowance: $3,000 and 3 CME days per year
All professional licenses covered, including malpractice
Phone allowance and company laptop
Paid bereavement (family and pets included)
Responsibilities
Perform comprehensive physical evaluations for an adult geriatric population
Train, educate and lead an interdisciplinary team, including advanced practice providers
Act as a medical liaison between the facility and others in the medical community such as specialists and hospital staff
Work collaboratively with an interdisciplinary team to ensure continuity of care
Prescribe all necessary medications and treatments
Qualifications
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
Board certification in Family Medicine, Internal Medicine or a related specialty
1+ year of experience as a physician, preferably within primary care or skilled nursing
