Purpose The purpose of this position is to provide comprehensive case management services that empower newcomers to successfully integrate into their communities. By delivering individualized support, connecting clients to essential resources, andfacilitatingaccess to higher levels of care when needed, the Case Manager plays a critical role in helping refugees achieve stability and long-term self-sufficiency.

Responsibilities

Provide in-depth case management services to refugees, SIV holders, and other ORR-eligible newcomers.

Collaborate withotherprogramteamto ensure coordinated and seamless service delivery.

Work with clients to develop individualized, achievable service plans and support them in navigating resources and programs to address barriers to resettlement and self-sufficiency.

Advocate on behalf of clients with local service providers and community agencies to strengthen referral systems and ensureequitableaccess to services.

Build andmaintainrelationships with health and mental health providers to expand access and improve culturally responsive care for clients.

Provide technicalassistanceand cultural competency trainings to service providers to enhance their capacity to serveatsen populations.

Explore and develop tailored programming and innovative opportunities to support families facing unique challenges in their resettlement journey.

Conduct regular assessments andmonitorclient progress to ensure goals are met, resources areutilizedeffectively, and service plans are updated as needed.

Organize workshops and group sessions for clients to promote stability, health, and wellness, with topics such asotheli literacy, stress management, self-care, women’s health, and family well-being.

Ensureaccurateandtimelycase file documentation and data entryin accordance withprogram and compliance requirements.

Travel regularly within the service delivery area for home visits, agency meetings, and provider coordination.

Actively participate in team meetings, training, and staff development activities to strengthen program delivery.

Perform other duties as assigned to support the Newcomer Support Program.

Qualifications

Bachelor’s degree or 3 years of relative work experience.

3-4 years’ experience working with unveil refugees or immigrants, and previous case management experience.

Other Skills

Strongly preferred that candidates are bilingual in English and a refugee language.

The successful candidate must be computer literate, with proficiency in MS Office Suite (Word and Excel) and internet applications.

Exceptional organizational and time management skills are required.

Excellent communication skills are essential, including an ability to communicate in a trauma-informed, positive, caring, and professional manner, demonstrating respect and empathy.

Ability to work in a multi-cultural environment required.

Special Requirements The job will require reference checks, standard criminal background checks, and motor vehicle background checks.
