Qualifications

Active, unencumbered license (LPN/LVN) in the state of practice

Compact licenses transferred within 90 days of state of residence

Current CPR certification (hands‑on component). Aveanna can assist with obtaining this after hire, if necessary.

TB skin test current within the last 12 months

Six months prior hands‑on nursing experience preferred but not required

Reliable transportation

Position Overview The Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care, under the supervision of a Registered Nurse, following the care plan and physician orders while maintaining confidentiality and professional boundaries.

Essential Job Functions

Deliver and coordinate quality patient care in compliance with physician orders.

Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.

Document all activities, assessments, nursing actions, responses, and coordination of care promptly.

Participate, implement, and update the nursing care plan.

Take appropriate nursing action based on assessment and achieve expected outcomes.

Recognize changes in patient needs and respond promptly to prevent or reduce risk.

Accept responsibility for personal and professional accountability, complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.

Provide care using infection control measures that protect staff and patient per OSHA standards.

Educate patient and family on disease process, self‑care techniques, and prevention strategies.

Maintain knowledge of competencies related to the nursing profession through educational programs, CEUs, internal learning management, and skill evaluations.

Requirements

Graduate of an accredited school of nursing.

Current, unrestricted state license as a Licensed Nurse in the state of practice.

Current CPR certification.

Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures.

Additional State‑Specific Requirements

South Carolina – One (1) year of pediatrics experience.

California – One (1) year of experience working under current nursing license.

Louisiana – One (1) year of experience as a licensed nurse.

Continuing education as required by state.

Preferences

Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.

Home health experience.

Reposition patients and move equipment without assistance.

Strong sense of smell and touch.

Respond physically and mentally to emergency situations in the home or during transport.

Function in various environments that may involve exposure to allergens and other conditions.

Possible exposure to blood, bodily fluids, and infectious diseases.
