A day in the life You’ll advocate for referred patients in the pre‑admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals by fostering relationships within medical facilities. With clinical expertise and problem‑solving skills, you’ll coordinate care plans with physicians and families, facilitate patient transfers, and address barriers to accessing specialised post‑ICU care.

Full‑time: Monday‑Friday 8 AM–5 PM

Key Responsibilities

Work with physicians, discharge planners, and patients’ families to plan the best continuum of care following an ICU or post‑surgical stay.

Perform clinical assessments of referred patients and counsel families to assist them in making informed care decisions.

Partner with hospital case managers and physicians to create a smooth pre‑admission and transfer process by removing payer‑related and operational barriers.

Required Skills

Registered Nurse (RN) or Respiratory Therapist (RT) license.

Ability to complete clinical assessments of medically complex patients.

Preferred Skills

Demonstrated success in patient advocacy.

Superior communication and relationship‑building skills.

Prior healthcare marketing or business development experience (extensive training provided).
