Job Overview
Residential Home Health and Hospice (‘Residential’) is looking to add to our extraordinary care team. Grounded by our belief that outstanding care is best delivered in a team‑based environment, our home health registered nurse case managers lead and collaborate to provide management and delivery of patient care plans. This position supports patients in Clarks Summit, PA and surrounding areas and offers a $10,000 first‑year success bonus for key achievements during the first year.

Qualifications
RN license in the state where you work
Graduate from an approved school of practical nursing
One year of RN experience with clinical competence to deliver quality patient care
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
Commitment to consistently meet critical deadlines for charting
Self‑management of time and schedule
Experience with tablets, mobile phones and EMR software
