Pay Range : $37.00/hr - $42.00/hr (actual pay based on skills and experience).
SUMMARY:
In need of an LPN or an RN responsible for providing direct nursing care to residents and supervising nursing assistants in accordance with federal, state and local standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Direct the day‑to‑day functions of the nursing assistants. Meet with assigned nursing staff and support personnel to plan the shift’s services, programs and activities. Provide written and oral reports/recommendations concerning shift activities. Admit, transfer and discharge residents as required. Provide leadership to nursing personnel assigned to your unit/shift.
Complete and file required recordkeeping forms/charts upon resident admission, transfer, and/or discharge. Receive and transcribe telephone orders from physicians and record on the Physician’s Order Form. Chart nurse’s notes in an informative and descriptive manner reflecting the care provided and resident response. Fill out accident/incident reports and submit to the Director as required. Perform routine charting duties according to established policies and procedures.
Prepare and administer medications as ordered by the physician.
Develop and maintain good rapport with inter‑departmental personnel and other departments to ensure nursing services can be maintained to meet residents’ needs. Create and maintain an atmosphere of warmth, personal interest and positive emphasis and a calm environment throughout the unit and shift.
Administer professional services such as catheterization, tube feeding, suction, dressing/bandage changes, colostomy, drainage bags, blood draws, massages and range‑of‑motion exercises as required.
Monitor assigned personnel to ensure compliance with safety regulations in the use of equipment and supplies.
Maintain confidentiality of all resident care information, including PHI. Report any incidents of unauthorized disclosure. Ensure all nursing care is provided with privacy and nursing staff knock before entering a resident’s room.
Provide IV and IV site care as needed.
Perform other duties as assigned.
QUALIFICATIONS
Registered Nurse with current unencumbered state licensure,
or
Licensed Practical Nurse with current unencumbered state licensure.
Must be able to read, write, speak and understand English.
Must be able to make independent decisions when circumstances warrant such action.
Must be knowledgeable of nursing and medical practices and procedures, and laws, regulations and guidelines pertinent to nursing care facilities.
Current CPR certification is required.
Experience in Long Term Care is preferred.
PHYSICAL DEMANDS
Move intermittently throughout the workday.
Cope with the mental and emotional stress of the position.
Meet general health requirements set by facility policies, including a medical and physical examination.
Push, pull, move or lift a minimum of 25 pounds to a minimum height of 5 feet and move such weight a minimum distance of 50 feet.
Assist in the evacuation of residents during emergency situations if necessary.
